All those trainees who need to apply, please complete this GP Trainee Declaration of Fitness to Practice document and return it to email@example.com. If you have already successfully applied online, please also complete the form for Health Education Wessex records. There is a 3 month grace period to end of April 2020, and you can check here: https://www.performer.england.nhs.uk/
Supported Return to Training for Academic Trainees
There are now new guidelines and FAQs from NIHR, with full details on the website. On return from your academic block, you will need a pre-absence meeting with your educational supervisor, 10-12 weeks before your planned absence, and complete a pre-absence form to be uploaded to your ePortfolio. This may also include an Individualised Action Planner (IAP). All queries to firstname.lastname@example.org.
